Central Vermont Home Health and Hospice is a not-for-profit agency committed to providing comprehensive, high-quality home health and hospice care to all Central Vermonters who need it, regardless of ability to pay. In addition, the agency promotes the general welfare of the citizens of Central Vermont with health promotion and long term care services. Service areas include, Waitsfield, Duxbury, Moretown, Fayston and Warren.
Programs and Services:

Home Care Program- provides intermittent, medically-necessary nursing and rehabilitative therapies in the comfort and privacy of your home. Referrals come through a variety of sources including physicians, hospitals, community
agencies, patients themselves or family members.

Hospice and Palliative Care Program- helps terminally ill people, together with their physician, family, friends, and clergy, face the end of life together. Offering a coordinated care program. Hospice works to enhance quality of life, even as death nears. Above all, it respects the right of terminally ill people to choose how and where they live and die. Bereavement support is available as well. Palliative Care focuses on symptom and pain management, with or without a terminal diagnosis.

Long Term Care Program- offers client-centered care for the frail elderly, and for permanently disabled individuals of any age. Provides assistance with activities of daily living and encourages independence, enabling clients to remain safely and comfortably at home.

Maternal Child Health Program- provides prenatal health care and postpartum home visits for infants and children with unique health needs.
Health Promotion Program- provides health education classes, blood pressure, and blood glucose checks, cholesterol screenings, flu and pneumonia vaccinations, foot care clinics, nutrition education, and more throughout the Central Vermont Area.

CVHHH has registered nurses on-call 24 hours a day, 7 days a week. Payment for home care comes from many sources including Medicare, Medicaid, private insurance, town funding, local business and individuals. Sliding fee scales are based on clients ability to pay
